For each day of the month, summary information concerning patients restrained (patient's name, type of restraint, time in restraint, ordered by, reason), medical therapies conducted (type of therapy, number of patients, time involved, comment), accidents (patient's name, nature of injury), tour of wards (time, ward, doctor's name, comment).

Three series of patient cards; each arranged alphabetically by surname.

Information includes name, patient number, date admitted, religion, social security number, birth date, date transferred or discharged. Some of the cards may be from the Minnesota Sex Offender Program [MSOP] which was created in 1993 to allow for the institutionalization of sex offenders after they had finished serving their prison sentences.

Researchers should check all three sets as it is unknown if they duplicate each other.
